What I heard

You want to walk further and sleep more comfortably, but you are unsure whether your hip, your back or the scan findings explain the problem.

My working explanation

In this fictional case, the pain around the side of your hip and the deeper ache after sitting appear to behave differently. One explanation may not account for both.

Why I think this

The pain at the side of your hip is brought on by stairs and lying on that side. The deeper buttock ache appears after sitting and eases when you get up and move. These different patterns suggest that more than one area may be contributing. The previous scan reports and temporary response to treatment are useful pieces of evidence, but neither settles the explanation on its own.

What remains uncertain

A remote consultation cannot provide every part of a hands-on examination. The contribution of each area remains uncertain, and this is a working opinion rather than a guaranteed diagnosis.

What happens next

The plan would address the two patterns separately and link progress to what matters to you: walking and sleep. If progress remains poor, an in-person reassessment may be more useful than repeating the same approach.

What would change the opinion

A different symptom pattern, new findings or a lack of progress would be reasons to reconsider the explanation. Your own report would include individual advice about when and how to seek further help.

An initial suitability conversation helps decide whether the service is appropriate. It is not an emergency assessment and does not replace advice from your GP, NHS 111, urgent care or emergency services.

The person behind Sherlock

Hi, I’m Andrew.

I’m an HCPC-registered physiotherapist with more than a decade of clinical experience. Since qualifying from the University of Salford in 2013, I’ve worked in advanced musculoskeletal practice and First Contact Practitioner roles, supporting people whose symptoms do not always fit a simple or obvious explanation.

But qualifications are only part of the picture. What matters just as much is knowing when to slow down, listen properly, question an earlier assumption and look again at the evidence.

I created Project Sherlock because too many people leave appointments with a scan result, a diagnostic label or a list of treatments—but still no clear understanding of what it all means for them.

My role is to bring the pieces together, explain the reasoning in plain English and be honest about what we know, what remains uncertain and what the next sensible step could be.

You do not need someone who promises all the answers. You need someone prepared to ask better questions.

Who is The Sherlock Review for?

Adults with persistent, complex or uncertain musculoskeletal symptoms who would value a considered specialist physiotherapy opinion. A short suitability conversation helps establish whether a remote review is appropriate for your situation.

How much does it cost, and what is included?

The Sherlock Review costs £199. This includes consideration of relevant information, one 60-minute remote consultation, a clinician-reviewed written opinion within two working days and one written clarification within seven days of receiving that opinion.

How do I ask whether the service is right for me?

Email hello@projectsherlock.co.uk to start a suitability enquiry. Please keep the first message brief and do not include detailed medical histories, scan reports, photographs or other sensitive documents by ordinary email.

What happens if a Sherlock Review is not suitable?

If the service is found to be unsuitable before booking or payment, there is no charge. If an appointment has already been booked and paid for and is subsequently found to be unsuitable, a 50% refund will be provided. Andrew may recommend a different type of assessment or appropriate healthcare professional.

Will I definitely receive a diagnosis?

No responsible clinician can guarantee a definitive diagnosis remotely. The Sherlock Review aims to help you understand what your symptoms and available evidence may suggest, where uncertainty remains and what next steps may be appropriate.

Do I need scans or previous reports?

No. Existing letters, scans and investigation reports can be helpful when relevant, but they are not required in every case. If documents are needed, you will be told how to share them appropriately after suitability has been established.

Is the consultation face to face?

The Sherlock Review is a remote video consultation. It is not a substitute for a hands-on examination where one is needed, and Andrew may recommend an in-person assessment if that would be safer or more useful.

When will I receive my written opinion?

Your personalised Sherlock Opinion will be reviewed and signed off by Andrew within two working days after your consultation, excluding weekends and bank holidays. Anything requiring immediate or urgent action will be communicated during the consultation.

What does the seven-day clarification include?

You may submit one written question to clarify something already covered in your Sherlock Opinion within seven days of receiving it. This does not include reassessment, a new consultation, substantial new evidence or ongoing messaging.

What if I need to cancel or rearrange?

Please provide at least 24 hours’ notice. A first cancellation with less than 24 hours’ notice can be rebooked once without penalty. Any further late cancellation or missed appointment is fully chargeable and non-refundable.

How will my information be handled?

This website is for public information and initial enquiries. Please do not send detailed health information through ordinary email. The full clinical privacy information and secure information-sharing arrangements will be explained before any clinical information is requested. A secure patient pathway is planned but is not yet available.

Can Project Sherlock help in an emergency?

No. Project Sherlock does not provide emergency assessment and does not replace urgent medical care, diagnostic testing or face-to-face examination where these are needed. If you have urgent, rapidly worsening or potentially serious symptoms, seek appropriate medical attention.
